We ask that you submit your application at least 7 days prior to your appointment. While we do our best to accommodate all requests, applications received with less than 7 days' notice cannot be guaranteed funding.
To verify your upcoming appointment, please provide proof in one of the following ways:
- Digital Medical Record: Take a screenshot showing the date and time of your appointment.
- Printed Documents: If you have a letter or other physical proof, feel free to take a photo. Please ensure that any personal information not relevant to this grant is covered for your privacy.
We are committed to making this process as easy as possible for you and welcome any suggestions or ideas that could further simplify it.
After gathering your proof of appointment, please email the image to efmpgrant@exceptionalmilitaryfam.com and include your full name, PayPal email address and phone number so we can match the information to your application.
For Emergency Requests
If you need urgent assistance, please send us a message via our Facebook page, and we will do our best to respond promptly. While we do our best to accommodate all requests, applications that are received with less than 7 days notice cannot be guaranteed funding.
Once your request has been approved, a member of the EFM team will reach out to you via email or text to notify you when the funds are being disbursed. Be sure to keep an eye out for our message!
