This initiative will provide adaptive equipment, therapy-related resources, and quality-of-life items that enhance the daily lives of EFMP families.
By expanding grant offerings for equipment and supplies up to $500 per request, this program ensures military families can access the necessary resources to improve their family member’s quality of life while alleviating financial hardship.
There is currently a $500 grant cap combined from any of our grants, per family member, per fiscal year.
If your family’s situation requires additional support, please share the details in the notes section. A member of our team will personally review your request for an exception.
Applications are processed in the order they are received. Should we run out of funding early, we will notify you via email. Current processing times is 90 days.
Verification of military affiliation
We must verify that you are the dependent of a US Service member.

Supporting Documentation for grant requests
EFM requires a brief letter of medical necessity, with the patient's name, and the item recommended by their provider (PCM, Specialist, OT, PT, Speech, ABA, etc.) on office letterhead with a handwritten signature or a military digital signature to process applications. If you do not upload this within 30 days, we will archive this application.

A testimonial is required after you receive you grant.
By applying, you confirm that you understand and agree to share a brief testimonial about your experience with this grant. These stories help us show the real impact of our programs and secure funding so we can continue supporting families like yours. If you prefer, we can share your testimonial anonymously. Only one testimonial is needed per family member who receives a grant, even if that individual receives more than one grant. Please note that continued eligibility for future grant opportunities may depend on submitting this feedback.
